To leverage expertise in evidence-based behavioral and environmental dementia treatment to provide professional education presentations to be delivered in ground rounds and ECHO sessions with an emphasis on referral and care pathways, as well as interdisciplinary best practices for managing dementia and common co-morbidities. Provides for the establishment of a roadmap for bi-directional referrals between community and clinical programs, with an emphasis on early diagnosis, support, and treatment.
